چکیده

BACKGROUND Childhood optic pathway gliomas (OPGs) are low-grade neoplasms intrinsic to the optic nerve, optic chiasm, tracts, and radiations. The management of OPGs is still a highly controversial topic among neuro-ophthalmologists. EVIDENCE ACQUISITION Authors' personal experience and literature review. RESULTS This review describes our current understanding of the behavior of OPGs and discusses advances in their imaging, evaluation, and management. Patients with OPGs that progress are typically treated with chemotherapy using carboplatin and vincristine; however, newer approaches to therapy are being explored. CONCLUSIONS Although chemotherapy is the mainstay of treatment when indicated, multicenter collaborative studies involving oncologists and neuro-ophthalmologists, both retrospective and prospective, are still needed to establish evidence-based guidelines for the management of children with OPGs.

Optic pathway gliomas: neoplasms, not hamartomas.

IMPORTANCE Optic pathway gliomas are an important neuro-ophthalmic cause of vision loss in children. Their management depends on whether they are considered neoplasms or hamartomas. OBJECTIVE To outline the evidence that optic pathway gliomas are slowly growing neoplasms and not hamartomas. DESIGN Review of relevant studies in the literature. SETTING The authors are from a pediatric terti...

Visual loss in children with neurofibromatosis type 1 and optic pathway gliomas: relation to tumor location by magnetic resonance imaging.

PURPOSE To examine the potential for visual acuity loss, and its relation to extent and location of optic pathway gliomas in a cohort of children with neurofibromatosis type 1 studied with magnetic resonance imaging. METHODS We reviewed the neuro-ophthalmologic records and brain/orbital magnetic resonance imaging scans for 43 consecutive pediatric patients with neurofibromatosis type 1 and op...

The cell of origin dictates the temporal course of neurofibromatosis-1 (Nf1) low-grade glioma formation

Low-grade gliomas are one of the most common brain tumors in children, where they frequently form within the optic pathway (optic pathway gliomas; OPGs). Since many OPGs occur in the context of the Neurofibromatosis Type 1 (NF1) cancer predisposition syndrome, we have previously employed Nf1 genetically-engineered mouse (GEM) strains to study the pathogenesis of these low-grade glial neoplasms....

Large hypothalamic and optic chiasm gliomas in infants: difficulties in distinction.

Hypothalamic and optic chiasm gliomas may be indistinguishable clinically, radiographically, and pathologically. Ten children with giant gliomas of the hypothalamus and optic pathway, all under age 2 years, had masses greater than 3 cm in diameter. Pathologically all proven cases (seven) were cytologically benign fibrillary astrocytomas. Previous authors have recognized the difficulty in distin...

Neurofibromatosis type 1 and sporadic optic gliomas.

AIMS To compare the natural history of sporadic optic glioma with those associated with neurofibromatosis type 1 (NF1). METHODS Optic glioma cases were identified using both the Manchester Children's Tumour Registry (CTR) and the North West Regional NF1 Database (NF1DB), with detailed information on natural history available from the former (in 34 of 36 cases identified). RESULTS A total of...

Optic Nerve, Chiasmal, and Hypothalamic Tumors

Gliomas are the most common tumors in the optic nerve, chiasmal, and hypothalamic regions of the central nervous system (CNS). As such, they are the focus of this chapter. For completeness, the less common tumors of these regions—meningiomas and craniopharyngiomas—are also covered. Germ cell tumors can also occur in this region but are discussed in Chapter 7. GLIOMAS Gliomas that affect the opt...
